There is a lot of room in HOW to report your child's information to your school district, and more information in regards to this can be found on Leah's website, www.nysleah.org , and also on the Home School Legal Defense Association's website, www.hslda.org .

Following are a few blank sample forms (not mandatory, just for an idea of what needs to be included):
